As registered physiotherapists, the physiotherapists at Avenue Physio are Custodians under Alberta’s Health Information Act (HIA). We collect, use, disclose, store, and protect your personal and health information in accordance with the Health Information Act (HIA) and other applicable privacy legislation.

We collect only the information reasonably necessary to provide physiotherapy and related health services.
Depending on the services you receive, this information may include:

  • Your name, address, telephone number, and email address;
  • Alberta Personal Health Number and billing information;
  • Emergency contact information;
  • Health history and medical information;
  • Assessment findings, treatment plans, and clinical records;
  • Referral and consultation information; and
  • Appointment history.

We may collect this information when you:

  • Book an appointment;
  • Complete registration, intake, or consent forms;
  • Receive assessment or treatment;
  • Communicate with us by phone, email, or through our website; or
  • Use our online booking or other electronic services.

Your personal and health information is used to:

  • provide physiotherapy assessment and treatment;
  • communicate with you regarding your care, appointments, and billing;
  • coordinate care with other health care providers involved in your care where
    appropriate;
  • process insurance claims, WCB claims, motor vehicle claims, or other authorized
    billing;
  • meet legal, regulatory, and professional obligations;
  • support quality improvement and clinic operations; and
  • provide other services authorized under the Health Information Act.

We collect, use, and disclose only the minimum amount of information reasonably
necessary for these purposes.

Avenue Physio maintains administrative, physical, and technical safeguards designed to protect your personal and health information from unauthorized access, use, disclosure, alteration, loss, theft, or destruction.
These safeguards include:

  • Secure electronic medical records through Jane Practice Management Software;
  • Encrypted cloud storage and secure backups;
  • Role-based access for authorized users;
  • Password protection and multi-factor authentication where available;
  • Confidentiality agreements for staff and contractors;
  • Ongoing privacy education and training; and
  • Regular review of our privacy and security practices.

We disclose personal and health information only where authorized by the Health Information Act or with your consent where consent is required.
Examples include:

  • Health care providers involved in your care;
  • Alberta Health Services where authorized;
  • Insurers, WCB Alberta, or motor vehicle insurers;
  • Electronic claims processing providers;
  • Approved Information Managers acting on our behalf; or
  • Where disclosure is required or permitted by law.

Avenue Physio does not sell your personal or health information.
